When the environment becomes damaged it is due to a failure of the market (Demand and Supply). That is, as the environmental damage has not been taken into account when applying the concepts of economic growth. This can generally be accounted for by the lack of property rights, as there is nobody who can take the side of the environment and hold to account those who damage the sea, air, rainforest etc, so there is very little done about it. In almost every circumstance it is cheaper for businesses/ firms to pollute the environment than to attempt to clean up the damage they have caused. Improving the balance of payments deficit also contributes to less damage on the environment, as there is again a cut in aggregate demand. The final aim of government policy is to redistribute income accordingly to a specific school of thought e.g. 1945-1979 The rich were taxed heavily (income tax) to pay welfare state (help the poor) which inevitably boosted aggregate demand, harming the environment. Since about 1970 the concept of sustainable development has been more widely discussed. Sustainable development is about economic growth being the cause of damage to the environment. An economist named R.K. Turner once quoted that sustainable development is â€Å"to leave future generations an amount of wealth, which is at least equal to that inherited by the current generation†. Agenda 21 set up in 1992 was an attempt to solve this world problem for the generations. It was a conference in Rio where all the countries governments agreed to an outline plan to protect the environment, especially global warming. To follow this attempt, a further conference was set up in 1997 in Kyoto. In this conference legally binding targets were introduced to reduce CO2 emissions with the exception of developing countries that were allowed to increase CO2 emissions up to a limit as they industrialise. In order to apply these individual targets set for the environment within a country would have to produce an evaluation to establish whether the benefits are greater than the costs to the environment before starting a project. This is done through Cost-Benefit analysis, which is a way in which economists assess the private internal costs and benefits (relates to the firm/ businesses) and the social external costs and benefits (relates to society). The problems that arise with this method of assessment though is that it is very subjective and open to wide interpretation, as you cannot value pollution e.g. an eye sore or the gains of a quicker journey to work. Solutions to pollution and other social costs can be approached in two ways, through the private sector and the government sector. I order to control pollution and social costs in the private sector without the interference of the government, several approaches would need to be made. Property rights would need to be extended, the polluters and the pollutee would need to bargain (Coarse Bargaining) about pollution, there might need to be a merging between the polluter and the pollutee so that it would be in the polutee’s best interest to clean up their act as the social costs would affect their profit levels. There might also have to be some altruism, which they can use as a marketing strategy. The price mechanism is also a strong solution to solving the problem of losing non-renewable goods e.g. oil. When demand exceeds supply then the prices rise and products like oil are instantly rationed which leaves an incentive to find alternatives. Alternatively for some products such as glass, paper, cans etc, it may become more economic to recycle as the prices for these scarce resources rise. However, i.e. with bottles, the factories used for recycling pollute the atmosphere as do the lorries collecting the bottles from the bottle bank as do the cars used by the public to take the bottles to the bank. Should the public sector fail to take these factors into account (market failure), there is a case for government intervention in order to apply a solution to pollution and other social costs. The government often sets standards as either an outright ban e.g. CFC’s or as a partial ban e.g. leaded petrol. Other standards are also introduced that are often more difficult to enforce as they are not as effective as taxes and the government may face possible costs e.g. MOT tests. Taxes are deemed as effective and are therefore often imposed in an attempt to internalise external costs. These are effective, as the buyer has to pay the equivalent price for the damage to the environment; this is an effective disincentive to buy e.g. VAT and excise duty on petrol. The rule that George Ohm came up with for working out the resistance of a wire is as followed: V/I = R V Volts I Current R Resistance This is known as Ohms Law Prediction The purpose of this experiment is to investigate the factors that affect the resistance of a wire. The hypothesis that I intent to prove or disprove is resistance will increase as the length of the wire increases, and resistance will decrease as the wire gets thicker. The experiment to come to this conclusion will be conducted as follows: firstly we attached three wires to nails that were held in place by a wooden board. The wires were attached o the nails by crocodile clips. The wires were all of different thickness, i. e. thick thin and medium circumferences. Electricity was supplied from a mains point,(in the form of volts) and from this high medium and low currents were passed through each of the wires. After all of the various currents had been passed through all of the wires, the resistance will be recorded using an ohm metre. After all of the initial measurements have been taken, the ohm metre will be moved 10 cm further away from the power supply for each of the wire, and will be further repeated in intervals of 10 cm to see if the length of the wire has any bearing on the resistance of the wire. All of the results will then be recorded in a table and then illustrated graphically to assist in the analysis of the end results. It is my prediction that as the current increases, and the length of the wire becomes longer; the resistance (measured in ohms) will increase as well. It is also my belief that as the wire gets thicker, the resistance will decrease. I also believe that the relation ship will prove to be linear and proportional between length and resistance . I devised this prediction after some careful and well thought out use of my scientific knowledge; the reasons I give for my prediction are as follows. The voltage travelling through the wires is a driving force, which effectively pushes the current around, acting as a kind of electrical pressure. A simple analogy to explain this would be to use the example of a hosepipe, i.e. a hosepipe with a greater circumference will offer less resistance than one with a smaller circumference, and also there is less surface area on a short piece of hosepipe, once again creating a smaller resistance. Also inline with my earlier analogy I believe that it will be a proportional relationship because if u double the length you double the available area for atoms within the wire The same principle can be used to explain resistance of a wire. The voltage is trying top push the current around the circuit, and the resistance is opposing it. The relative size of the voltage and resistance decide how big the current will be. So therefore if you increase the voltage, more current will flow, or more voltage will be needed to keep the same current flowing. So as a consequence I believe that a long thin wire, and the smallest will offer the greatest resistance by a thick short wire. The GMO it is always a topical theme, on television, on radio, in newspapers, often we talk about this topic so debated and even painful contrast. But before expressing their own opinion and their own thinking, we must understand what it means and what are GMOs. The abbreviation GMO is the achromic of terms: genetically modified organisms. This are non-human organisms whose genetic material has been altered by genetic engineering techniques. The first GMO of the modern era, was obtained from S. N. Cohen and H. Boyer that were able to clone a frog gene. GMOs are produced through biotechnological processes. Biotechnology are techniques that exploit the properties of both plant and animal cells to produce new varieties of plants or animals. Biotechnology is not an invention of recent decades. Simply the process of fermentation of the wine, which is achieved by exploiting some bacteria, is a biotechnology process known since ancient times. In the years between 1997 and 2010 the area of land cultivated with GMOs had a huge increase, we have moved 4.2 million to 365 million hectares. In fact, 10% of the planet’s arable land is used for genetically modified crops. Most of these crops are located in North America, but it should be registered, in recent years, a rapid increase in acreage in so-called developing countries. In the USA in the years 2009/2010, the vast majority of soy (93%), cotton (93%), corn (86&) and sugar beet (95%) have been grown with genetically modified varieties. Data for 2012 published by the International Service for the Acquisition of Agri-Biotech Applications (ISAAA) show that in Europe there has been a 13% increase of cultivated area (globally, the increase was 6%). Politics is always a tough thing to talk about, to follow, to express and even to educate yourself about. In the essay written by George Orwell â€Å" Politics and the English Language† he deliberately expresses that writing of today isn’t the same writing of yesterday. In other words, George Orwell expresses his thoughts; today we have developed habits both bad and good, which can challenge truth behind the text and can manipulate the reader’s understanding. As I came across the article ‘No plan B for pentagon in case of big budget cut† by Thom Shaker, an article which seems more for show than for information, seems to glorify the pentagon and the military. Numbers and statistics can easily be manipulated and played with so that the reader unless intuitive, has a hard time questioning, believing anything in print. The article states that if the white house and congress fail to an agreement there will be massive budget cuts, and the defense department has yet to plan for it. George Orwell would classify this article to be an example of good writing. George Orwell says to be simple and clear, almost as if so that the reader does not have to overcome any obstacles to understand the information thus presented. The writer while busy glorifying the pentagon and military, also states the issue spot on which was earlier mentioned of how the defense department has not planned for possible budget cuts, and soon enough the author backs it up and supports the issue or popular question presented. The author states that the reason behind not planning was to prevent preparedness and provide security, so that the congress does not invent to not agree with the White House. No specific jargon words, which Orwell mentions to dismiss in his essay(pg.3) There are no â€Å" bad habits† exhibited throughout the article, the author does a well done job at keeping it simple and clear at the same time as sophisticated and presentable, a young adult has the same understanding as a matured adult. No mumbo jumbo to â€Å" deficit, manned and unmanned, contingency† Comprehendible, at a a broad demographic  range. George Orwells quote from his essayâ€Å" but merely language as an instrument for expressing and not for concealing or preventing thought†. Keep it at a simple level, to not persuade a reader by their maturity of understanding from either side, matured adults vs. young adults. Through Orwell’s â€Å" rules of writing†you can truly understand how societies â€Å"bad habits† have influenced the writing of today. The improper use of techniques to seem sophisticated has lead to nonsense and confusion. Political writing always seems to be nonsense and confusion, unless you follow and do your own research the truth always seems to be manipulated. A. Assessment of Patient The assessment of Mrs. Baker should include vital signs including pulse oximetry. Given her difficulty in breathing, lung sounds should be auscultated. Because she is on two different medications that could affect blood pressure, lisinopril and hydrochlorothiazide (HCTZ), hypotension could be one cause of her collapsing. In addition to vital signs, decreased peripheral pulses and capillary refill can also be indicators of hypotension. Also, because HCTZ is a diuretic, dehydration should be considered. Since she was in her backyard when she collapsed, it could be that she was doing yard work. Thus, it is important to know what she was doing before she collapsed. The outside temperature and whether Mrs. Baker became overheated or drank enough fluids could be useful factors in assessing her fluid-electrolyte balance. Serum electrolyte levels and vital signs should be taken, and color and skin turgor should be assessed. Dehydration could also contribute to hypotension. Mrs. Baker should be placed on a cardiac monitor and an EKG obtained to rule out any dysrhythmias. Given Mrs. Baker’s respiratory symptoms and her history of hypertension and diabetes (which are the two biggest risk factors for congestive heart failure), a chest x-ray should be done. Arterial blood gases could assist in further assessing her oxygenation, as well as acid-base balance. Also, since Mrs. Baker is a diabetic, a blood glucose level should be done, since hypoglycemia could be a cause of her mental status changes. When Mrs. Baker is alert, pain can be assessed through a verbal â€Å"numbers† scale, or a â€Å"faces† scale if she is mildly confused. When she becomes unresponsive, non-verbal indicators of discomfort should be observed, such as muscle tension, grimace, and grunting. B. Technological Tools A stethoscope is an essential assessment tool. It is needed to assess breath sounds, heart sounds, and perhaps assist in taking the blood pressure. A stethoscope works by amplifying internal sounds through a diaphragm and transmitting them through a hollow tube to the earpieces. (Schunk, p. 1) The sounds can be interpreted by a skilled assessor and used to make a diagnosis and prescribe treatment. In Mrs. Baker’s case, it would be extremely important to listen to her lung sounds to determine if there are changes associated with atelectasis, or more likely, to assess for fluid sounds in the lungs, which would be an indicator of congestive heart failure. If a manual blood pressure measurement is being taken, the stethoscope would also be essential to listening for restored peripheral blood flow as the pressure in the cuff is being released. The benefit of a stethoscope is that it is a simple tool that can be used to easily obtain an initial assessment of a patient’s breath sounds while waiting on more complicated assessments, such as blood gases and chest x-rays. If a manual blood pressure cuff is not available, blood pressure will probably be obtained using an electronic blood pressure device such as a Dinamap. A blood pressure measurement, as well as other vital signs, would be key in determining whether Mrs. Baker was suffering from hypotension related to her new medication, lisinopril. A manual blood pressure cuff has the benefit of being more accurate at extremely high or extremely low pressures, while an electronic device, such as a Dinamap, can be set to automatically check blood pressure at regular intervals. Many cardiac monitors also have blood pressure capabilities built in, so that may be an option for obtaining blood pressure. A cardiac monitor would be used for Mrs. Baker to watch for arrhythmias, which could be caused by lisinopril, or be another sign of congestive heart failure or an acute myocardial infarction. Mrs. Baker has a history of hypertension and diabetes, which are the two greatest risk factors for heart disease. An electrocardiogram (EKG) would be useful for diagnosing heart disease or damage as well. A cardiac monitor is essential in continuous monitoring of the patient, but a 12 lead EKG has the benefit of being able to better pinpoint the specific areas of the heart that are affected or damaged. Blood sugar can be obtained from a drop of blood with a glucometer such as One Touch, or it may be obtained along with the serum chemistry and electrolyte analyses from the lab. A glucometer is a quick and easy way of determining the blood glucose level. Mrs. Baker is a diabetic, and is on an antidiabetic drug, Metformin. Her change in mental status could be due to low blood sugar. This would be easily diagnosed with a glucometer, and easily treated with IV glucose, so it should be one of the first assessments made. A glucometer has the benefit of being a quicker and easier way to obtain a blood sugar level, while a serum glucose may be more accurate at extremely high or low levels. Pulse oximetry can be done with a dedicated pulse oximeter, or it may also be built in to the capabilities of the cardiac monitor. Since Mrs. Baker is suffering from respiratory difficulty, it is essential to find out her blood oxygen saturation level. An arterial blood gas would not only be helpful in giving a more precise measurement of arterial oxygen and carbon dioxide, it would also give useful information about the pH of her blood and the bicarbonate level. Pulse oximetry has the benefit of alerting caregivers to acute changes in blood oxygenation level, while arterial blood gases gives a more detailed picture of the of the state of gas exchange in the lungs Serum electrolytes would be measured by the laboratory. These would be helpful in diagnosing dehydration and/or kidney function. Electrolytes are also essential in cardiac function. In fact, serum electrolytes are a good overall way to assess homeostasis in a patient. A radiographer will use an X-ray machine to obtain the chest x-ray, either in the radiology department, or will use a portable model. A chest x-ray is another way to assess respiratory function. A chest x-ray could show fluid in the lungs, as well as atelectasis or infiltrate. It would also show cardiomegaly, which is another indicator of congestive heart failure. A portable chest x-ray would be beneficial, as it would not require Mrs. Baker to be taken from the emergency department. Finally, if Mrs. Baker has been a patient at this facility before, the Electronic Medical Record can make it easy to find information on her past medical history. This is an improvement over medical records of years past, which usually had to be located in extensive file cabinets or even on microfiche. Sometimes these records were lost or misfiled. Electronic Medical Records are usually easy to locate. C. Data Collection Prioritization Vital signs, pulse oximetry, and a visual assessment should always be the first assessments performed on a patient. The severity of the vital signs will determine whether emergency life-support interventions need to be initiated at once, and will give the caregivers clues to what needs to be looked at next. Since hypoglycemia is an emergency situation, a finger-stick blood sugar should be the next assessment formed. And since Mrs. Baker is having respiratory difficulty, breath sounds should be assessed as soon as possible to determine what measures need to be taken to insure adequate ventilation. Lab work, x-rays, and other tests should be done after the initial assessments, since the results will take longer to obtain. D. Pain Assessment Comparison In alert and oriented patients with no cognitive impairments, the best way to measure pain is to use the â€Å"numbers† scale. Using this scale, you have the patient rate their pain on a scale from 0 to 10, with 0 being no pain, and 10 being the worst pain they can imagine. Pain is subjective, meaning that it can only be judged by the person in pain, and not by an observing party. The numbers scale gives an easy way for a person to tell her nurse the level of her pain, and to judge how much it is relieved by interventions. A geriatric patient who is alert but has mild cognitive impairments may be confused by numbers, but can be given the Wong Baker faces pain scale to help to measure their pain in the same way as the numbers scale. A patient who is not alert or who is unconscious is obviously not going to be able to describe her pain to her caregiver. In this case, the caregiver will have to observe the patient for non-verbal signs of discomfort. These include grimace, muscle tension, increase heart rate, respirations, or blood pressure, and moaning or grunting. E. Pain Management If Mrs. Baker was not alert enough to respond to questions but was showing signs of pain, and given the standing orders of acetaminophen or morphine, I would elect to give the patient morphine. The patient is not alert enough to take any medication by mouth, and an oral medication will take longer to work. Intravenous Morphine works very quickly, and may decrease the work of breathing in patients with congestive heart failure. The caregiver should reassess the patient’s pain after giving any pain medications. In the case of a non-alert patient, the caregiver would look for signs such as stabilization of vital signs and muscle relaxation. I have learned that the geriatric patient has a decrease in physiologic reserve, and therefore there is a shorter time between changes in homeostasis and a state of shock. Because of this, when an elderly patient is showing signs of distress, assessment and interventions should be done quickly to improve outcome. I also learned that an elderly patient may live their everyday life with a certain level of chronic pain, so when they complain of acute pain, it should be taken seriously and treated promptly. F. Team Members Mrs. Elli Baker may have been assessed initially by an EMT or paramedic, if emergency medical services had been called to transport her to the emergency room. Upon arrival, she would have been assessed by a nurse, and at the same time or shortly thereafter, the emergency room physician. The nurse would have obtained vital signs, applied the cardiac monitor, and obtained the finger stick blood sugar, while the physician would have given an initial set of orders. A respiratory therapist would have probably been called, given her respiratory difficulties, and an EKG technician would have been called to do an electrocardiogram. Labs may have been drawn by a phlebotomist or a nurse, and the arterial blood gases would have been obtained by the respiratory therapist or a phlebotomist, depending on the facility’s policy. Finally, a radiology technician would have performed the chest x-ray. When the nurse noticed that Mrs. Baker became unresponsive and started having more difficulty breathing, she would have notified the physician of the changes and called any necessary support staff to assist with Mrs. Baker’s immediate care.
